How to use First Responders Resupply Form:

  • Click on the First Responder resupply Forms button from the homepage

  • First Responders Resupply homepage will show any previously created entries

  • To create a new record click on the blue NEW EMS RECORD button in the upper right hand corner.

  • FD/EMS Service Name: If you have already created the Unit as a “Destination” and designated their type as “First Responder” they will appear in the drop down menu.

  • Person Placing the Order: Contact person at the EMS/FD Company

  • FD/EMS Staff Name: Person receiving the order from the pharmacy

  • Pharmacy Staff Name: Person filling the order

  • Enter the drug name or NDC and select the correct product.

  • The corresponding pedigree information will auto-populate.

  • Complete the ”Ordered Quantity” and “Recorded Quantity”

  • Click on “Add Additional Drug” if necessary

  • Click “Save Drug” when complete

  • The corresponding pedigree information will auto-populate.

  • Complete the ”Ordered Quantity” and “Recorded Quantity”

  • Click on “Add Additional Drug” if necessary

  • Click “Save Drug” when complete

First half of the newly created Resupply form

  • If product needs to be added at the last minute, the user can click on +ADD DRUG button.

  • Order form will show newly added product.

Second half of the newly created resupply form

  • Click on the SAVE FORM button

  • Navigate to the top of the page and click PRINT to generate a paper copy.

  • Have Pharmacy staff and person picking up the order sign the final copy.

  • Click on BACK TO RESUPPLY FORMS button
